Welcome to our home school. We are excited to offer exceptional teaching and learning for your child(ren). Please complete this registration form by August 1st and return to Dr. Zenon Education Services to secure your spot. Only 10 spots are available. Courses are held Monday-Thursday 8am - 12Noon. - Technology Fee

- All Supplies (pens/pencils/markers, workbooks, folders, paper, etc) are included for registered students.
- Does NOT include any admissions or exit testing.
Agreement
TEST. The school required new students or a transferee provide testing from previous school for admission to the Wise Owl Scholar Academy.

INTERVIEW. The school administrator will conduct an interview with the parents and student as part of the screening.

REQUIREMENTS. It is important to complete all requirements before school starts. Failure to do so may delay the student from coming to school.

MEDICAL CLEARANCE. The school requires all students to have a yearly physical exam. For new students, medical clearance from their doctor is required.

BEHAVIOR. A certificate of good moral is required in order to get admitted to this school. Please request it from the previous school as soon as possible.
