List your top 3 designs you'd be interested in as 1st choice, 2nd choice and 3rd choice. Most of my pre-drawn designs I only tattoo once, though I now have a few repeatable pieces. Exception can be made if two pals want to have matching pieces for a design not listed as repeatable.

Desired size in inches*
Give a range if you are unsure! 2 inches is my minimum for most designs, with some small exceptions. The more detail the piece has the larger it's minimum size will be.
Desired placement(s) ** you can be specific: I.E. inner left ankle, right forearm, arm, lower leg etc *
We can play around with stencils on the day of your appointment to make sure you are 100% certain on placement! I do not tattoo: large/colour rib pieces, necks, behind ear, intimate areas, palms, hands, feet & sides of fingers.
If you have other tattoos in your desired placment we'd need to work around : attach a photo of your desired placement(s) : feel free to edit your desired design(s) onto your desired placements!

I have read and understand Erika Jeanne's Booking FAQ, either on her website or through the saved instagram story highlight on her profile.*
The booking FAQ is where you can find my hourly rate, things I'm interested in tattooing and other basic info.
If contacted, I understand I have 2 days to pay my non-refundable deposit. Paying the deposit means I agree to the price quote & understand it is only for the date & time offered. Deposits are $57.50 for flash and $115.00 for custom projects.*
I understand Heirloom only allows one client per artist present, NO ADDITIONAL GUESTS are allowed in the procedure area while the client is being tattooed. Matching appointments will be on separate days or back to back time slots not at the same time.*
I am comfortable with Erika Jeanne using the requested pre-drawn flash design for merchandise, such as, but not limited to: stickers, prints, clothing, pins. Please note some designs are currently in use.*
