VEX Competition Volunteer Application

We are excited to host the upcoming VEX IQ Rapid Relay Tournament at San Marino Robotics, and we need your help to make this event a success! We are seeking enthusiastic volunteers to assist with various tasks throughout the day.

Event Details:

Date: 12/15/2024 & 2/2/2024
Location: 3100 Huntington Drive, San Marino, CA 91108
Volunteer Check-In: 8:30 AM
Event Conclusion: 4:30 PM

Volunteer Roles:

Field Reset: Help ensure the competition fields are ready for each match.
Team Check-In: Assist with registering teams and guiding them through the event.
Judging Support: Work with our judges to facilitate interviews and review engineering notebooks.
Scorekeeping: Help record and manage scores during the matches.
General Assistance: Provide support wherever needed to keep the event running smoothly.

No prior experience is necessary—we will provide all the training and guidance you need on the day of the event. This is a great opportunity to get involved in the local STEM community, support young innovators, and be a part of an exciting competition.

PARENT PERMISSION AND WAIVER 家長許可和棄權

We, the parents/guardians of the student named below, acknowledge and understand the nature of the classes and activities taking place at 2549 Huntington Dr Suite 101, San Marino, CA 91108. By signing this form, we give our full permission for our child to participate in these activities, knowing that appropriate supervision will be provided at all times.

In the event that medical attention is required due to an injury, we authorize the supervising teachers or staff (including volunteers) to care for our child. Should further medical treatment be necessary, we expect to be contacted for specific authorization before any action is taken. However, if attempts to reach us are unsuccessful, we grant permission for the necessary medical care to be administered.

Additionally, we authorize the supervising teachers or staff (including volunteers) to take our child to a physician, dentist, or hospital in the case of a serious accident or illness, if we cannot be immediately located.

We understand and accept that unforeseen situations may arise during any trip or activity, whether school-sponsored or otherwise, that are beyond the reasonable control of the supervising teachers or staff (including volunteers). In light of this, we agree to release and hold harmless Leducate LLC, its Directors, agents, officers, employees, and volunteers, from any and all liability, claims, demands, suits, judgments, costs, interest, and expenses (including attorneys’ fees and costs) related to these activities. This release includes, but is not limited to, accidents or injuries sustained by the student, medical expenses incurred, and any causes beyond the control of Magikid Lab San Marino, such as natural disasters, civil unrest, acts of terrorism, or war.
