I am aware of the dangers and risks of attending and participating in gymnastics, dance, ninja challenge and related activities. I understand that serious injury, or even death may result during participation in any activity offered by Sterling Academy of Gymnastics. I hereby give consent for myself, or the above mentioned student, to the coaches and staff of Sterling Academy of Gymnastics to obtain medical care from any licensed physician, hospital, or clinic, for the above mentioned student(s), for any injury that might arise during participation in any of the programs of Sterling Academy of Gymnastics. I promise not to hold Sterling Academy of Gymnastics, its officers, agents, coaches, or employees liable or pursue legal action against them. This waiver absolves Sterling Academy of Gymnastics of any duty for injuries sustained on the premises before to, during, or after the activity.
By signing this agreement, I agree to hold Sterling Academy of Gymnastics completely harmless, including financial responsibility for any injuries sustained, regardless of the cause or circumstances.
I will do all in my power to obey Sterling Academy of Gymnastics staff and all safety requirements, and I will seek clarification if necessary.
