Data Protection Act 1998 - The personal information collected on this form will be used by Dr Fazil Kucuk Turkish School. This information is being collected so that we put your child in the right class for their age group and so that we can track their attendance and progress. It also helps us to keep your child safe - so that we can contact you in an emergency. All information will be held centrally and confidentially.

Bu bilgiler çocuğunuzun yaş gurubuna uygun sınıfa yerleştirilmesi, katılım ve gelişmesini takip etmemiz için alınır. Bu bilgiler ayrıca çocuğunuzu güvende tutmak için bize yardımcı olur. Örneğin herhangi olası bir acil durumda sizinle irtibata geçebiliriz. Bu bilgiler, Dr Fazıl Küçük Türk Okulu dışarısında (çocuk koruma yasasına aykırı bir durum olmadığı sürece) başka bir kuruluş ile paylaşılmayaktır.

Dr Fazıl Küçük Turkish School aims to maintain high standards of education and to provide a safe and professional learning environment.

Your co-operation in taking active part in your child(ren)'s education, especially in supporting us and encouraging home learning is crucial to ensure that your child(ren) find the learning enjoyable and achieve their learning goals.

• I (parent) will be responsible for ensuring attendance and on time of my child to Dr Fazıl Küçük Turkish School and to online lessons when face to face lessons are not available.
• I (parent) understand that if my child is attending a Crèche classroom, I must always stay in the school premises with him/her. I (parent) will be responsible for ensuring that my child adheres to the required behaviour rules at the school.
• I (parent) give permission for my number to be enrolled to a WhatsApp group for the purposes of communicating with the class only and agree not to use the group for any other purposes.
• I (parent) will ensure my child completes his/her homework.
• I (parent) will be responsible to provide study material and essential equipment for my child such as Pencils, rubber, pens etc.
• I (parent) will inform, Dr Fazil Kucuk Turkish School / Head of School as soon as possible if my child is unable to attend a lesson.
• I (parent) will be responsible for the behaviour of my child.
• I (parent) understand that I am responsible for payment of all fees, to the school's accountant. There is NO REFUND if I/my child does not want to attend the School any more.
• I (parent) understand that full payment of the school fee must be made by the 15th of December set by the school committee. If the full payment is not made, School committee has the right not to admit the pupil until the payment is made, unless special arrangements have been made between parent and Head of School.  
• Any child under 11 years old must be accompanied to and from School unless permission is given by the parent/ carer of the child to return home unaccompanied, by filling the Permission Form Under 11's.

SECTION 8:  Photography/Images Consent Form / Fotoğraf ve görsel kullanma kabulu

Dr Fazil Kucuk Turkish School is committed to the protection of children and vulnerable adults involved Dr Fazıl Küçük Turkish School activities.

Staff/trustees acting on behalf of Dr Fazil Kucuk Turkish School sometimes take photographs or video footage for publicity purposes. These images may appear in our printed publications, on our website, on our Facebook page, other social media etc. Before taking any pictures, we need your permission. Please answer the questions below, then sign the form where shown.

1.The school will not include details or full names of any child or adult in an image, on our website, or in printed publications without good reason and additional consent.

2. The school will not include personal e-mail, postal home address or telephone number(s) on our website, printed publications, Facebook, or any other social media account etc.

3. If the School uses photographs of individuals, the name of that child or young personal will not appear in the accompanying text or photo caption unless we have your additional agreement.

4.If the school names a child or young person in the text, we will not use a photograph of that child to accompany the article.

5. The school may use group images with very general labels, such as 'making decorations', 'children receiving awards'.

6. The school will only use images of children who are suitably dressed to reduce the risk of such images being used inappropriately.

7. As the child(ren)'s parent/guardian, you agree that if you take photographs or video recordings of your child(ren) that include other children or young people you will use these for personal and family use only. Where consent has not been obtained from the other parents for any other use there would be a breach of the Data Protection Act 1998 if images were used for any wider purpose.

The information that you provide will be used by Dr Fazıl Küçük Turkish School as data controller in accordance with the General Data Protection Regulation and Data Protection Act 2018.

We will process all personal data fairly and lawfully.

We will only process personal data for specified and lawful purposes.

We will endeavour to hold relevant and accurate personal data, and where practical, we will keep it up to date.

We will not keep personal data for longer than is necessary.

We will keep all personal data secure.

We will endeavour to ensure that personal data is not transferred to countries outside of the European Economic Area (EEA) without adequate protection.

• Tüm sınıf calışmalarımı ve ödevlerimi zamanında ve elimden gelen en iyi şekilde tamamlayacağım. (Complete all my class work and homework on time and to the best of my ability)

• Başkalarına karşı kibar ve yardımsever olacağım. (Be polite and helpful to others)

• Okulun çevresine, ekipmanlarına ve mobilyalarına saygı göstereceğim. (Respect the school environment, equipment and furniture)

• Hiçbir derste cep telefonu kullanmayacağım. (Not use mobile phones in class)
