We are excited to introduce the inaugural Community and Family Builders (CFB) Annual Survey! At CFB, our mission is to educate and empower residents in Pennsylvania to tackle the critical issues affecting the health and welfare of minority and working-class families. From addressing health disparities like Alzheimer's in communities of color to promoting peaceful alternatives for violence prevention, we are dedicated to building stronger, healthier communities. 

This year's survey will focus on caregiver programming, an essential aspect of our commitment to supporting families facing health-related challenges. Your insights will help shape future programs, ensuring that we meet the needs of caregivers and families in our communities. We value your input as we work together to create meaningful, impactful solutions that empower our neighborhoods for generations to come.

Responses are anonymous and confidential. A report will be created by Transcendworx, a third-party consultant to ensure anonymity.

A caregiver supports another person in meeting their daily needs. Their responsibilities can include tasks such as bathing, dressing, feeding, providing transportation, shopping for groceries, doing housework, managing incontinence, helping with mobility, preparing meals, administering medications, and coordinating with medical professionals.
