This agreement is entered into between Life Raft Parent Coaching, LLC and the above named client.
COACH WILL:
- Provide parent coaching services via phone or Google Meets.
- Provide summaries of key points covered during each session and sometimes offer suggested ‘homework’ to be considered between sessions.
- Provide information, resources, support, and affirmation to Client.
- Maintain confidentiality (exceptions include client sharing an intent to harm self or others).
- Bring knowledge, honesty, and qualified experience.
CLIENT WILL:
- Make themselves available for scheduled sessions.
- Bring honesty, engagement, openness and commitment to change.
BILLING:
Client will be billed $150.00 for each session. Client will start with three sessions for a total of $450.00. A payment link will be sent via email and payment is due before the first session.
CHANGING SCHEDULED COACHING SESSIONS:
If the client wishes to change/cancel a coaching session, they will communicate via phone, text or email a minimum of 24 hours prior to the scheduled session to avoid being billed for a missed appointment. Both parties will make every effort to communicate about conflicts that would necessitate scheduling changes.
CONFIDENTIALITY:
Information revealed by the Client during any communication with the Coach is confidential and will not be shared with any outside party without Client’s consent. Exceptions to the standard are:
- Suicidal statement, which shall be shared with family members and appropriate mental health professionals, if needed.
- Statements of intent to harm others, which will be reported to law enforcement personnel and to the potential victim/s.
- Evidence of child/spouse/elder physical or sexual abuse that will be reported to the appropriate State agency according to State Law.
- In response to a lawfully issued subpoena, provided that Coach will give notice to client of the subpoena in order to give Client an opportunity to take steps to prevent compliance, if client so chooses.
INTENT OF COACHING:
The Client acknowledges and explicitly understands that services being delivered by the Coach in no way should be defined or construed as professional counseling, psychiatry, social work, therapy and/or any additional service that requires professional licensure at the County, City, State or Federal levels. The Client agrees not to hold the Coach liable for any acts or omissions in the delivery of coaching services. The Client understands that they are always free to reject any suggestions, request, or advice that is offered by the Coach. The Coach will never willfully act in a manner that is harmful or dangerous to the Client. In the event that the Client believes they require the services of a licensed counseling professional, the Client is solely responsible for that determination and subsequent efforts to find such a professional and seek treatment.
TERMINATION:
The Client or the Coach may terminate the Coaching Agreement for any reason at any time. If Client terminates agreement, Coach shall keep remainder of balance on any payment. If Coach terminates agreement, Coach will refund remainder of balance to Client.
MODIFICATION:
This agreement may not be changed, modified or amended unless mutually agreed upon by both parties in writing.
GOVERNING LAW:
The laws of the State of Colorado where Coach resides. This document constitutes the entire agreement and understanding of both parties and any and all prior agreements, understandings, or representations, are hereby terminated and canceled in their entirety.
