2024-2025 CCD/Sacrament Registration Form

Religious instruction for our children takes place every Wednesday evening from 6:45-7:45pm, from October 2nd to mid-May. To enroll your child/children, please fill out the following form. If you have a child/children of the proper age and disposition to receive a sacrament, but cannot attend regular classes, please continue this form and fill out the relevant information. We will reach out to those families to ensure we remain in communication as sacrament time approaches and be available for any support you may request for catechesis at home.

Confession and Confirmation begin at or about age 7. First Holy Communion begins at or about age 8. Families must be registered at OSGA and attend Mass regularly there (regularly = about 3-4 Sundays per month and Holy Days).

Fees & Payment Processing

The cost for CCD classes is $25 per child, but a maximum of $100 per family
Fees can be paid online or by check. If you choose to pay online, please proceed to this LINK to process payment. Checks may be submitted in Sunday's collection basket. Please be sure to write CCD on the envelope and add "CCD" in the memo line. Thank you.
