Premier Pickleball Brand New Player Form

This form is for adding a BRAND NEW PLAYER to the PPL league - someone who has never played in the PPL before at all - AND to establish correct rating information.

NOTE - All information provided below will be verified via any source that provides free public access. The club pro may also be consulted before the new player is approved for play & given a BENCHMARK rating &/or STARTING DYNAMIC rating.

These measures are taken to help ensure the integrity of this ratings based league.
