Parent/guardian(s) of a dancer under the age of 6 that require assistance with going to the washroom MUST sign permission in order to receive help from a member of The Landing Dance faculty or administrative staff. By checking this box and signing below, you, the parent/guardian of said enrolled dancer, authorize a member of The Landing Dance Centre Faculty, Faculty Assistant or Administrative Staff to assist my dancer in the washroom while they are at the studio or at an event organized by the studio, should yourself or another parent/guardian be unable to assist.
