Church Nomination Instructions:

You are allowed one nomination per active 50 members or part thereof. Example: 53 active members = 2 nominations. If you nominate more than allowed only the first ones will be counted. IE: If your church is eligible for 2 nominations and you submit 3, only the first 2 will be counted. 

Individual Nomination Instructions:

If you are an ordained minister and married, you and your spouse get one nomination each. Your spouse can either fill out this form with you, or if they have their own Individual ID, they can complete a separate form. If you nominate more than allowed, only the first ones will be counted. IE: If you are eligible for 1 nomination and you submit 3, only the first will be counted.
