Criteria For Hall Of Fame For Professional Drivers:

1. Driving for a PMTC member company for the past 12 months on a full time basis (min. 40 hours/week)

2. Practices and promotes safety on the job

4. Recognized by peers as outstanding in his/her field

5. Has a preventable accident free driving record (indicate nominees record below) 

Note: Nominees must qualify for the "Years and Kilometers" or "Years and Hours"of the following three accident free driving criteria:

• Attach a letter (300 words or less) telling us what makes this driver special, and how he/she practices and promotes on the job safety, including any commendable actions or awards received, any volunteer community activities he/she is involved with, and anything else the panel should consider in reaching a decision.

• By submitting this nomination form the person named below confirms that the candidate named above meets all the criteria and qualifies for consideration by the judging panel for the Hall of Fame for Professional Drivers and that the candidate named above has been notified of this entry and certifies all information submitted is correct.
