This Bonfire Groove is for grown folk 21 and up. "We're gonna have us a FUNKY GOOD TIME." The vibe is Southern Soul / Juke Joint. Wear your suits and sequence dresses, cowboys boots and jeans, costume or white tank with a toothpick. Come as you like, just make it funky. The atmosphere is all love mixed with laid back fun. Enjoy Live Music, Soul Food, Dancing, Open Bar, Networking, Entertainment and more. (Keep in mind, chairs and tables will be on grass.)

RELEASE OFLIABILITY AND Waiver CONSIDERING the risk of injury that exists while participating in “BONFIRE GROOVE” (hereinafterthe “Activity”); and CONSIDERING my desire to participate in said Activity and being given the right to participate in same; I HEREBY, for myself, my family, my heirs, administrators, executors, assigns, or personal representatives (hereinafter collectively, “Releasor,” “I” or “me,” which term also include Releasor’s guardian or parents if Releasor is under 18years of age), voluntarily and knowingly enter into the WAIVER AND RELEASE OFLIABILITY and hereby waive any and all rights, claims of cause of action of any kind arising out of my participation in the Activity; and release and forever discharge City County Lifestyle, Bomax Ranch and Retreat, Take Flight 21 Inc, and KnoBe Lifestyle collectively known as “We Live 4 This” hereinafter referred to as “The Parties” located at 617Lee’s Lake Road, Fayetteville, GA 30214 their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors, and assigns (collectively “Releasees’), from any psychological or physical injury that I may suffer as a direct result of my participation in the aforementioned Activity. I AM VOLUNTARILY PARTICIPATING IN THE AFOREMENTIONED ACTIVITY AND I AM PARTICIPATING IN THE ACTIVITY AT MY OWN RISK ENTIRELY. I AM AWARE OF THE ASSOCIATED RISKS WITH PARTICIPATING IN THIS ACTIVITY, WHICH MAY INCLUDE BUT ARE NOT LIMITED TO: PHYSICAL OR PSYCHOLOGICAL INJURY, ILLNESS,PAIN, SUFFERING, DISFIGUREMENT, TEMPORARY OR PERMANENT DISABILITY (INCLUDING PARALYSIS), EMOTIONAL OR ECONOMIC LOSS, AND DEATH. I UNDERSTAND THAT THESE INJURIES OR OUTCOMES MAY ARISE FROM MY OWN OR OTHERS’ NEGLIGENCE, CONDITIONS RELATED TO TRAVEL TO AND FROM THE ACTIVITY, OR CONDITIONS AT THE ACTIVITY LOCATION(S). NONETHELESS, I ASSUME ALL RELATED RISKS, BOTH UNKNOWN AND KNOWN TO ME, OF MY PARTICIPATION IN THE ACTIVITY. I FURTHER AGREE to defend, indemnify, and hold harmless the Releases against any and all suits, claims, or actions of any kind whatsoever for damages, liability, compensation or otherwise brought against by me or anyone on my behalf, including attorneys fees and any related costs. I FURTHER AGREE that I do not have any injury, physical disorder, or disability that might jeopardize my participation; and that I assume all risks or any damage, injury, loss, or disability to my person or property that may occur as a result of such participation. I fully and completely ACQUIT, RELEASE, AND FOREVER DISCHARGE “The Parties’ its respective principals, directors, agents, officers, employees, insurers, representatives, successors, and assigns, from any and all actions, claims, and causes of action that I now may have or may have in the future of any nature whatsoever. I FURTHER ACKNOWLEDGE that Releases are not responsible for errors, omissions, acts or failures to act of any party or entity conducting a specific event or activity on behalf of Releasees. In the event that I should require medical care or treatment, I authorize “The Parties” to provide all emergency medical care deemed necessary, including but not limited to, first aid, CPR, emergency medical transport, the use of AEDs, and sharing of medical information with medical personnel. I further agree to assume all costs involved and agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance. I FURTHER ACKNOWLEDGE that this Activity may involve a test of a person’s mental and physical limits and may carry with it the potential for death, serious injury, and property loss. I agree not to participate in the Activity unless I am medically able and properly trained, and I agree to abide by the decision of the “The Parties” official or agent, regarding my approval to participate in the Activity. I HEREBY ACKNOWLEDGE THATI HAVE READ THIS “WAIVER AND RELEASE” CAREFULLY AND UNDERSTAND FULLY THAT IT ISA RELEASE OF LIABILITY. I EXPRESSLYAGREE TO RELEASE AND DISCHARGE “The Parties” ANDALL OF ITS MEMBERS, AGENTS, AFFILIATES, MANAGERS, ATTORNEYS, STAFF, VOLUNTEERS,HEIRS, REPRESENATIVES, SUCCESSORS, PREDECESSORS, AND ASSIGNS, FROM ANY AND ALLCLAIMS OR CAUSES OF ACTION AND I AGREE TO VOLUNTARILY GIVE UP OR WAIVE ANYRIGHT THAT I OTHERWISE HAVE TO BRING A LEGAL ACTION AGAINST The Parties” FOR PROPERTY DAMAGE OR PERSONAL INJURY. I FURTHER ACKNOWLEDGE that to the extent that statute or case law does not prohibit releases for ordinary negligence, this release is also for such negligence on the part of “The Parties” agents, and employees. I agree that this Release shall be governed for all purposes by law, without regard to any conflict of law principles. This Release supersedes any previous oral or written promises or other agreements. If any damage to equipment or facilities occurs as a result of my or my family’s or my agents’ will-fill actions, neglect or recklessness, I acknowledge and agree to be held liable for any costs associated with any such actions of neglect or recklessness. MEDIA RELEASE: I grant permission for City Country Lifestyle LLC, Bomax Ranch and Retreat, KnoBe Lifestyle, and Take Flight 21 Inc., known as “We Live 4 This” hereinafter referred to as “The Parties” and associated entities to use photos, videos, sound, likeness, or any other media taken during the event for promotional and any other purposes deemed necessary. I, the adult participant or adult signing on behalf of a child hereinafter known as “Signee”, hereby grant permission to “The Parties” their representatives, and employees their revocable and unrestricted right to use and publish photographs, audio recordings, and/or video recordings of me, taken during any event, program, or activity organized or sponsored by for editorial, promotional, and advertising purposes, in any format or medium, including but not limited to print, online, social media, television, and radio. I understand and agree that these materials may be used without any further consent or notification and without compensation to me. I also understand that “The Parties” may share these materials with third parties for the same purposes. I waive any right to inspector approve the finished materials before they are published or distributed. I hereby release, discharge, and hold harmless “The Parties,” their representatives, and employees from any and all claims, liabilities, damages, or demands arising out of or in connection with the use of the materials, including but not limited to any claims for defamation, invasion of privacy, or infringement of moral rights. I represent and warrant that I am of legal age and have the right to enter in to this agreement. If I am under the age of majority in my jurisdiction. THIS WAIVER AND RELEASEOF LIABILITY SHALL REMAIN IN EFFECT FOR THE DURATION OF MY PARTICIPATION IN THE ACTIVITY, DURING THIS INITIAL AND ALL SUBSEQUENT EVENTS OF PARTICIPATION.
