Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Cookin Around The World during the workshop. In exchange for the acceptance of said child’s candidacy by Cookin Around The World, I assume all risks and hazards incidental to the conduct of the activities, and release, absolve, and hold harmless Cookin Around The World, and all its respective owners, staff, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected workshop activities.

In case of injury to said child, I hereby waive all claims against Cookin Around The World, including all staff and affiliates, all participants, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all cooking activities, including field trips. Some of these injuries include but are not limited to, the risk of fractures.
Photo and Video Permission

I hereby grant Cookin Around The World permission to use photographs and/or videos of my child taken during the workshop for promotional purposes, including but not limited to social media platforms, the company’s website, and marketing materials. I understand that these images and videos may be used without further notification or compensation.
