This application is for financial support to receive subsidized rates for counseling services if you are unable to pay the full fee due to financial constraints or other circumstances.

To qualify for subsidized rates for counselling services, please submit ONE of the following documents for verification:

1) Last 6 months CPF contribution history

2) Last 3 months' payslips

3) Latest Notice of Assessment (NOA) from Inland Revnue Authority of Singapore (IRAS)

4) Student pass or matriculation card for full-time students (with clear expiration date shown)

5) Public Assistance Card (not CHAS card) 

6) Eligibility for other financial assistance programs (such as government benefits or community support services)

Terms & Conditions:

• Eligibility: To qualify for subsidized rates, applicants must demonstrate financial hardship or eligibility for other financial assistance programs. Documentation must be provided to support claims of financial constraints.

• Application Requirements: Applicants must submit ONE of the following documents:

• Subsidy Limit: RANGE Counselling Services Pte. Ltd. will provide a subsidy towards counseling fees but will not cover 100% of the costs. The amount subsidized will be determined based on the submitted documentation and financial need. For example, instead of the full $180/session rate, the subsidised rate might be $100/session.

• Approval Process: All applications for subsidized rates are subject to review and approval by RANGE Counselling Services Pte. Ltd. The company reserves the right to request additional information or documentation as needed.

• Confidentiality: All submitted documents and information will be handled with strict confidentiality and used solely for the purpose of determining eligibility for subsidized rates.

• Reevaluation: Subsidized rates are subject to periodic reevaluation. Applicants may be asked to provide updated financial information if their circumstances change.

• Non-Transferability: Subsidized rates are non-transferable and apply only to the individual applicant. They cannot be shared or transferred to another person.

• Changes and Cancellations: RANGE Counselling Services Pte. Ltd. reserves the right to modify or cancel subsidized rate agreements if the applicant’s financial situation changes or if the terms and conditions are not met.

• Dispute Resolution: Any disputes regarding the subsidized rates will be addressed on a case-by-case basis by RANGE Counselling Services Pte. Ltd., whose decision will be final.
