  • Recent changes have been made to the definition of an independent contractor. These updates may affect how you are classified and how we work together.

    To ensure that you understand and agree with these changes, we kindly ask you to complete the attached Independent Contractor Check form.

    By selecting AGREE for all the questions, you confirm that you understand and agree that you are an independent contractor under the new definition.

    As an independent contractor, if you answer DISAGREE to any questions you need to stop work immediately and reassess your relationship with Made To Help.
