PBGC/CYC requires the submission of several forms in order to participate in the tour, including but not limited to: Medical Form, Consent Form, Government Form DS3053 (notarized and signed by both guardians), and an original copy of the participant's birth certificate (guardians may apply for a certified duplicate copy). I agree to submit all forms in a timely and complete manner, and I understand that failure to do so may result in being unable to participate in tour.

CYC is able to offer partial financial aid to families who submit an application and qualify. (Note: you must apply to receive financial aid for tour *even if* you are currently receiving financial aid for choir tuition). Do you intend to apply for financial aid for tour?

The total price for South Africa is $5,750. A non-refundable deposit of $750 is due on October 15, 2024. 8 equal payments of $625 are due on the 15th of each month between November and June. If you cancel before January 13, 2025, $3500 is refundable. If you cancel before March 30, 2025, $2000 is refundable. Cancellation after May 1, 2025 is non-refundable. I understand this payment structure and agree to complete all payments in a timely and accurate manner.
