PBCC requires the submission of several forms in order to participate in the tour, including but not limited to: Medical Form, Consent Form, Government Form DS3053 (notarized and signed by both guardians), and an original copy of the participant's birth certificate (guardians may apply for a certified duplicate copy). I agree to submit all forms in a timely and complete manner, and I understand that failure to do so may result in being unable to participate in tour.

PBCC is able to offer partial financial aid to singers 18 or younger who submit an application and qualify. (Note: you must apply to receive financial aid for tour *even if* you are currently receiving financial aid for choir tuition). Do you intend to apply for financial aid for tour?

The total price for Ghana is $4,680. A non-refundable deposit of $750 is due October 15th. 8 payments of $491.25 are due between November and June on the 15th of each month. Cost of visa and trip insurance (recommended) are not included. Triple accommodations are included; double supplement is $130 per person. If cancellation occurs before 2/13/2025, the refund is $3,180. If cancellation occurs between 10/2/2024 and 2/13/2025, the refund is $2,430. If cancellation occurs between 2/14/2025 and 5/6/2025, the refund is $1,100. After 5/6/2024, no refund is available for cancellation for any reason. I understand that trip cancellation insurance is not required, but it is recommended. I understand this payment structure and agree to complete all payments in a timely and accurate manner.
