1. Purpose
The purpose of the Patient Portal Consent Form is to get consent from patients who will use the Patient Portal to access, view and monitor their medical conditions, book GP appointments online, making backend regular medication script requests, accessing immunisation records, allergy records, and lab results (once they have been noted by the GP).
2. Patient Portal
The patient can access the secure Patient Portal via the internet.
3. Terms and conditions
The patient can access the Patient Portal via his or her credentials and it is the patient's responsibility to protect the credentials from unauthorized people, services or organizations.
a) Please do not book consults for Driver’s or any other License, Medical Reports or Medical Records requests, Fitness & Insurance Health Checks via the portal. Please call to book these via reception.
b) All booking of appointments must be done responsibly. Duplicate reservations for purposeful blocking of appointment slots may lead to cancellation of your portal access or online booking privileges.
c) Cancellation of appointments policy: for appointments booked by phone, cancellation can only be made by calling the reception. For appointments booked through portal, cancellation can be done directly via portal. Any cancellation done in less than 24 hours of appointment time would be fully chargeable unless your slot is taken up by another patient.
d) Your online portal access will be terminated whenever you are no longer enrolled with our Clinic.
e) One email address can be used only for one patient to sign on to the portal. Patients less than 16 years old are not allowed to sign onto their own portal access. However, parents can book under their own name in the portal when booking GP appointments, and put a comment/ remark – booking for my child (Name).
4. Risks
The medical information, personal information and communication channels in the Patient Portal are protected securely. However, the patient needs to be careful while using the portal.
