This Release and Waiver of Liability executed on (today's date) in favor of Greenhouse Ministries of Murfreesboro, TN, (Greenhouse) a non-profit organization, and its directors, officers, employees and agents.
The Volunteer desires to work as a volunteer for Greenhouse and engage in the activities related to being a volunteer thereof, The Volunteer freely, voluntarily and without duress executes this release under the following terms:
1. RELEASE & WAIVER: Volunteer releases and forever discharges and will hold harmless Greenhouse, its directors, officers, employees, agents, and its successors and assigns from any and all liability, claims and demands of whatever kind or nature either in law or in equity, which arise or may hereafter arise from Volunteer's activities with Greenhouse. Volunteer understands that this release discharges Greenhouse from any liability or claim that the volunteer may have against Greenhouse, its directors, officers, employees and agents with respect to any bodily injury, personal injury, illness, death or property damage that may result from "Volunteer" activities with and/or at Greenhouse, whether or not caused by the negligence of Greenhouse or its officers, directors, employees or agents.
Volunteer also understands that Greenhouse does not assume any responsibility for or obligation to provide financial assistance or other assistance, including, but not limited to medical, health or disability insurance in the event of injury or illness of Volunteer.
2. MEDICAL TREATMENT: Volunteer releases and forever discharges Greenhouse from any claim whatsoever which arises or may hereafter arise on account of any first aid treatment or service rendered in connection with the Volunteer's activities with Greenhouse. In case of emergency, I give permission for the administration of medical treatment or medication for me and do further agree to the performance of such treatment, anesthetics, and surgery as deemed necessary by the attending physician for me.
3. ASSUMPTION OF RISK: The Volunteer understands that the activities include work that may be hazardous to the Volunteer, including, but not limited to: loading and unloading; carrying and lifting; and the use of ladders or step stools. Volunteer expressly and specifically assumes the risk of injury or harm in the activities and releases Greenhouse, its directors, officers, employees, and agents from all liability for injury, illness, death, or property damage resulting from Volunteer's activities.
4. INSURANCE: The Volunteer understands that Greenhouse does not carry or maintain health, medical or disability insurance coverage for any volunteer.
5. PHOTOGRAPHIC RELEASE: Volunteer grants and conveys unto Greenhouse all right, title and interest in any and all photographic images and video or audio recordings made by Greenhouse during the Volunteer's activities with Greenhouse.
IN WITNESS WHEREOF, the Volunteer has executed this release as of the date written above.
