We understand that choosing a primary care provider is an important decision, and we are honored that you have entrusted us with your healthcare needs.
As part of our commitment to providing high-value healthcare services in a safe and compassionate setting, we have implemented a few essential guidelines to help ensure our patients and employees remain safe while visiting our facilities, and in our communication with you, our patient. We have recently introduced our new Patient Behavior Code of Conduct, included on the second page of this letter, which outlines our mutual responsibilities in maintaining a respectful and collaborative patient-provider relationship. We believe that effective communication and mutual respect are the cornerstones of excellent healthcare, and we ask that you, and any family members or caregivers review and respect this code of conduct while visiting our clinic or hospital.
We have also enclosed your New Patient Forms for your convenience. Please complete these forms before your initial visit. Forms can be completed online using our fill-form process. If you don’t have access to a computer, you may print, complete and mail them to our office, or drop them by our clinic, (Monday – Friday 8am – 5pm). Your completed forms will assist our dedicated healthcare professionals in providing you with personalized attention, expert medical guidance, and comprehensive support every step of the way.
A Release of Information form is also included to allow you to transfer your medical records to us. Please complete this form online for the fastest processing of your records transfer by our Healthcare Team. If you don’t have access to a computer, you may print, complete and mail the forms to our office to have appropriate healthcare records forwarded to our Healthcare Team to review. If you have questions about completing the Release of Information form, please contact our Health Information Management team at (541) 396-1058 (8am to 5pm, Monday – Friday).

Behavior Expectation Agreement
Coquille Valley Clinic has a strong tradition of excellence in patient care. We are committed to providing safe patient and family centered care along with the patient’s participation.
These expectations outline our partnership agreement which is intended to provide compassionate care in an environment that promotes comfort, healing, and mutual respect between the patient and the clinic team.
Expectations of the Patient and Clinic Partnership Agreement:
- Patients, family, visitors, and all members of the Clinic Care team will work together in a respectful environment to provide the best possible care for the patients. A respectful environment includes all forms of communication: phone, face-to-face, written, and in the patient portal. Rude, threatening, demeaning comments or behaviors will be called out by the care team. Care may be suspended temporarily if any clinic employee feels uncomfortable, but resume when respectful behavior/communication is exhibited. Security will be called to intervene if negative behaviors continue after requests have been made to stop.
- Any threatening behavior, including physical threats, demonstrated by the patient may result in the termination of care by the clinic. Clinic employees will immediately contact security or law enforcement.
- Coquille Valley Clinic has a zero tolerance for Workplace Violence including abusive actions or language, or any other behavior that creates a risk or threat to patients, families, visitors, or clinic employees. Anyone, including family members, violating our zero-tolerance policy will be asked to leave the Clinic property. Clinic employee may call security or law enforcement immediately.
