Screening Protocol

New clients must fill out and submit this application, so I can determine whether we are a good match.

Before I will see you for our first time, I require: 

• A picture of you and a picture of your driver's license, sent to my private email address.  You will receive that address in my reply to this application.
• A compensated coffee meeting before booking. When we meet, you will pay for the entire session up front. I accept Venmo or Apple Pay. 

Tardiness / Cancellation Policy

If you are late, I will be compensated for our entire scheduled time.  I may allow a 10 minute grace period at my discretion.

You must reschedule at least 48 hours in advance in order to transfer your deposit to your new appointment. 

If you cancel with less than 24 hours notice, you will forfeit your deposit.  In addition, I will require 50% of the session tribute for you to remain in good standing. 

My Limits

Any activity that is sexual, illegal, unsafe, or non-consensual.

Health concerns

In-session BDSM activities may bring up signficant bodily, mental, and emotional stress.  Please describe your current medical conditions, so Mistress X can determine which activities should be modified. 

Protocol

First, we will meet over coffee so I can interview you to see if we are compatible.  If we have compatible interests, I will send you a follow-up email with a link to schedule a session.

Statement of Informed Consent

I am requesting and at my own risk participating in a BDSM session. I understand activities within the Session will be conducted by mutual consent. I accept sole responsibility to:

1. Decide what activities I wish to participate in during the Session and decide what my boundaries are.
2. Communicate these decisions clearly before the start of the Session.
3. Agree to an appropriate safe word or other signal to request an activity stop.
4. Immediately say or signal any unexpected fatigue, shortness of breath, chest pain, nausea, or discomfort.

I understand I can discontinue participation at any time during the Session, without prejudice against me, beyond fees or tribute for the Session.

Limits of Liability

I understand that any activity undertaken during the Session is voluntary and is at my own risk. I agree to indemnify and hold harmless all participants in the Session from all claims of liability, injury, death, property loss or damage, attorney’s fees and costs, and I agree to reimburse these participants for any such claims they may have or incur.

Agreement

By signing below, and submitting this session application, I acknowledge I have read, understood, and agreed to the contents of this form, and I confirm that all information given by me is correct to the best of my knowledge. I understand there is neither sex nor sexual acts involved in any Session.  I am a consenting adult age at least 21.  I am not a law enforcement officer nor a federal law enforcement agent.
