*South Carolina Youth Lacrosse - MEMBERSHIP REFUND POLICY*
Your purchase of a South Carolina Youth Lacrosse membership provides your child with immediate insurance coverage and covers costs for coach and officials training, field reservations, scheduling and other adminstrative expenses. All membership purchases are considered FINAL once submitted and NO REFUNDS will be provided.
*RELEASE OF LIABILITY FOR MINOR PARTICIPANTS*
READ BEFORE ACCEPTING
IN CONSIDERATION OF my child/ward, being allowed to participate in any way in South Carolina Youth Lacrosse (SCYL) and its affiliates and related events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. The risk of injury and illness to my child/ward from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, FOR MYSELF, SPOUSE, AND CHILD/WARD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my child/ward's participation; and,
2. I warrant that my child/ward is in good health, has no condition or defect which would interfere with his/her participation; and,
3. I willingly agree to comply with the program's stated and customary terms and conditions for participation. If I observe an unusual significant concern in my child/ward's readiness for participation and or in the program itself, I will remove my child/ward from the participation and bring such attention of the nearest official immediately; and,
4. I for myself, my spouse, my child/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS South Carolina Youth Lacrosse and its affiliates; its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to
conduct the event ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property incident to my child/ward's involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASES OR OTHERWISE, to the fullest extent permitted by law.
5. I, for myself, my spouse, my child's/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releases from any and all liabilities incident to my child's/ward involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.
6. I, the parent/guardian, assert that I have explained to my child/ward: the risks of the activity, his/her responsibilities for adhering to the rules and regulations, and that my child/ward understands this agreement.
I, FOR MYSELF, MY SPOUSE, AND CHILD/WARD, HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT WE HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
*BRAIN INJURY / CONCUSSION RISK MANAGEMENT PLAN *
If your child/ward sustains a blow to his/her head or any other injury during the season, whether during an SCYL event or outside of this program, you must report it to the coach.
If your child seeks medical attention for any injury, including a concussion, you must get a medical clearance form from an MD stating that your child/ward has been cleared for full activity and vou must present this to your child's ward's coach prior to return to participation.

I have read, understand, and agree with the refund policy as stated above.
I have read, understand, and agree with the waiver/release agreement above which applies to injuries and communicable diseases. I have also explained this waiver/release to my child ward including the risks of participation and he/she understands this risk and his/her respective responsibilities for adhering to the rules and regulations and accepts them as a participant.
I have read, understand, and agree to adhere to the South Carolina Youth Lacrosse Brain Injury / Concussion Risk Management Plan and my requirements for notification, to seek medical treatment, and to obtain written medical clearance before my child/ward can return to activity.
I have read, understand, and agree to adhere to the MBYLL Child Abuse Risk Management Plan.
**CODE OF CONDUCT**
Players, coaches, officials, parents and spectators are to conduct themselves in a manner that demonstrates respect to other players, coaches, officials, parents, spectators and fans. In becoming a member of this organization, an individual assumes certain obligations and responsibilities to the organization and its participants. The essential elements in this Code of Conduct are HONESTY and INTEGRITY. Those who conduct themselves in a manner that reflects these elements will bring credit to themselves, their team and their organization. It is only through such conduct that our organization can continue to earn and maintain a positive image and make its full contribution to our sport and community.
The following essential elements of the Code of Conduct must be followed:
Sportsmanship and teaching the concepts of fair play are essential to the game and must be taught at all levels and developed both at home and on the field during practices and games
The value of good sportsmanship, the concepts of fair play, and the skills of the game should always be placed above winning.
The safety and welfare of the players are of primary importance.
Players should always demonstrate positive behavior and respect toward teammates, opponents, coaches, officials, parents and spectators.
Coaches, players, parents and spectators are expected to demonstrate the utmost respect for officials and reinforce that respect to players/teammates.
Grievances or misunderstandings between coaches, officials or any other parties involved with the organization should be communicated through the proper channels and procedures, never on or about the field of play in view of spectators or participants.
Spectators involved with the game must never permit anyone to openly or maliciously criticize, badger, harass or threaten an official, coach, player or opponent.
Eligibility requirements, at all levels of the game, must be followed. Rules and requirements such as age, previous level of participation, team transfers, etc, have been established to encourage and maximize participation, fair play and to promote safety.

I hereby grant South Carolina Youth Lacrosse, its directors, coaches, agents, and affiliates the right to publish and display photographic and or video images of my child participating in South Carolina Youth Lacrosse programs and activities for the purpose of promoting South Carolina Youth Lacrosse and the achievements of its participants. I waive all rights to fees and compensation for the use of same.
