    • An additional itemized statement of funeral goods and services selected will be emailed to you immediately after submission.
    • Charges are only for those items that you selected or that are required. If we are required by law or by a cemetery or crematory to use any items, we will explain the reasons in writing. A rigid, combustible container is required by the crematory. A permit for cremation is required by the state.
    • We charge you for our services in obtaining additional death certificates (if applicable). Our charge for obtaining cremation permits is included in your package.
    • DISCLOSURES
      If you selected a funeral that may require embalming, such as a funeral with viewing, you may have to pay for embalming. You do not have to pay for embalming if you did not approve it. If you selected arrangements such as direct alkaline hydrolysis, direct cremation, or immediate burial, we have charged for embalming, we will explain why below.

      ACKNOWLEDGEMENT AND AGREEMENT
      This funeral home is authorized to perform services, furnish goods, and incur cash advance charges specified in this Statement. It is also acknowledged that a General Price List was received.

      TERMS: Cash advance items are those charges made by other agencies outside our funeral home for various items (i.e., flowers, cemetery expense, obituaries, memorandums, luncheon expense, church fees, etc.). These items, along with any other cash advance items, are due on the day arrangements are made. This is a cash transaction. Total amount is due in full and, in all events, becomes PAST DUE AND DELINQUENT upon expiration of the due date. A penalty of 18.00% per annum for UNANTICIPATED LATE PAYMENT will be charged 10 days after the cremation takes place. Purchaser(s) agree to pay reasonable attorney’s fees, court costs, and other costs of collection incurred in the collection of this debt. I/We having read the above, accept and approve same, and jointly and severally promise to make full payment thereof. Receipt of a copy of this contract is acknowledged.
