15-week Group in the Invitation to Change Approach

Stress Management | Communication Tools | Trust Building

In this group, participants will use the Invitation to Change Approach to reflect on what it means to help, reimagine the role that loved ones can play in the change process, and learn evidence-supported strategies for helping a loved one struggling with addiction.

Space for this group is limited! Registration is on a first come, first serve basis, with priority given to parents and guardians of LaGuardia high school students, as well as parents and guardians of other NYC high schoolers.

I understand that by registering for and participating in this CMC:FFC ITC peer led group that I will respect the confidentiality of my fellow group members. No personal information that is shared in this group shall be shared outside of the group. It is important for members to feel they can speak, if they want, about personal experiences or feelings that they may have some shame, fear, or extreme worry about. When speaking about their experience with their loved one, they are sharing not only their own story, but the story of their loved one who is not here to give or refuse consent. This is why we must pledge to hold in confidence any identifying information about who is present in the group and whomever they are related to.

Treatment Referrals*

I understand that CMC:FFC will not provide treatment referrals for group participants or their loved ones. CMC:FFC offers trainings to family members and mental health professionals to help them work with folks with substance use issues. Next Steps!

After you submit this form you will receive a link to our pre-group survey. This will ask you a series of questions about yourself and your loved one. We will ask you the same set of questions after you complete the group. This feedback will help us continue to track the effectiveness of these groups and to improve the experience of future group members. In order to finalize your registration, you must complete this survey. Once you complete the survey you will receive a free print copy of our Invitation to Change: A Short Guide our workbook for families which will be used throughout your group.
