Wedding Ceremony Forms & Required Documents

The Catholic Archdiocese of Edmonton
At least six (6) weeks before the wedding, the wedding parties meet with the priest for the canonical interview and complete the required documentation, including the following documents:

1. Certificate of Baptism for the Catholic party/parties, issued for marriage purposes after the initial interview.

2. A copy of the baptismal certificate of the non-Catholic party.

3. Proof of freedom (Form 2A) & Concerning Baptism (Form 2B) from your parish (if from another parish)

4. Certificate of Marriage Preparation Course (www.trinitycatholic.net/marriage)

5. Permission letter from parish priest (if from another parish)

6. Marriage License (from Registry Office) Complete legal names of two (2) witnesses with their respective complete mailing addresses (with postal codes) and Phone Numbers.
