  • After submitting this form, Sam wil review the information and your appointment time/services/pricing may be adjusted or increased based on your current hair/goals detailed in this form. 


    Your appointment in salon will begin with a 10-15 minute full consultation to refine the plan for service and solidify pricing for that day's service. 


    Generally, new guests are able to be seen within 1-3 weeks of desired appointment times, but guests with limited availability may have to wait up to 4 weeks to be seen. 

     
    CLIENT: "I understand that if I need to cancel my appointment for any reason, I will let Sam know as soon as possible, out of courtesy. However, if I 'No Show or No Call' an appointment, I may be banned from booking future appointments with the salon."
