VENDOR AGREEMENT

The Shack System Martial Arts, Ikonik Youth, GSGV Events & those contracted to coordinate selected event above are not responsible for any sales, negative outcome, theft, injury or death to myself, property/business or those in attendance with me. All participants on site (vendors & attendees w/my business) at 765 W. Rte 66, Glendora CA 91741 waive all liability from said entities listed above and I elect that I am participating as a vendor at my own risk under my own discretion upon signature and completion of this application. I am aware no electricity will be provided and I am responsible for my trash clean up. I am aware there may be a duplicate/similar category as mine. Narcotics, Alcohol, Weapons are not permitted on site. I am aware I may have my participation revoked and will be asked to leave the property.
