Reason the children are leaving the premises & transported: Vacation Care Excursion

Method of Transport: Bowie Nugent (Assistant Coordinator) or Jeffrey Mate (Educator) will be driving a 22 seater budget buses with seat belts. Bowie is a holder of a LR licence for a 22 seater bus and Jeffrey Mate is a holder of a HR licence.

Children will be transferred in groups via mini bus . Children attending in Years 3+ on 8/1/25 and 17/1/25 will have access to the main pool water inflatable - competence and decision to use this will be assessed for each child by Swimming Pool staff on arrival.

AGREEMENT

By ticking the consent box below and signing this form, parents/guardians agree to the following :

• I agree to notify the service if my child is not attending their booked session.
• I understand that if my child/ren do not arrive before the start of their group times for excursions, they will miss out on the experience.
• I understand that my child will be attending an excursion off site, as per the travel details listed above if booked for the excursion. Any changes to these details will be communicated immediately.
• I have discussed with my child/ren the importance of following educator directions & instructions (any child displaying inappropriate behaviour will result in a review of ability to attend further excursions).
• I understand there is no refund for cancellations of excursions or assisiated bus charges from time of booking if notice is not given by the assigned cut off date located on the program.
• I will ensure my child/ren wears the appropriate clothes to excursions (including YELLOW shirt and closed in shoes)
• I will ensure my child/ren has packed appropriate safety atire for activities where required (e.g. Helmet).
• I confirm that the service has my child's updated information (e.g. medical/contact)
