  • Employee Safety & OSHA Agreement

  • As a clinical facility offering treatment to a wide range of individuals, it is important for Rehab 2 Perform employees to recognize the importance of Safety and Confidentiality. Occupational Health and Safety Administration (OSHA) requires that employers a workplace free of known health and safety hazards. OSHA guidance states that the employees should take care to ensure not only the safety of the clients being worked with but more importantly personal safety. Personal safety starts with what is on the employee. While in the clinical setting, it is required that employees wear attire as outlined in the company dress policy. This supports the ability to move freely, comfortably and safely while working with clients. It also ensures that the employees’ risk from injury is mitigated. Employees should follow other safety related guidelines for activities such as lifting and use of hazardous chemicals/materials. Employees should also be mindful to work with another employee or assistive device when the weight of an item or person exceeds 50lbs. If using potential hazardous items, be sure to read the labels for guidance on usage.

    In addition to a physical Safety covered by OSHA, it is important to remember personal safety for the employee and clients. Rehab2Perform meets with clients in its facility’s open working space. This is to ensure transparency. For the protection of all involved, Rehab2Perform employees should not assist clients alone, in areas not openly visible or in spaces such as bathrooms. If a client does require assistance in a private area, this should be done only with another employee or a client’s representative present. In an emergency always immediately contact emergency medical professionals.

    Finally, confidentiality is essential. All client information should be handled with care and concern for privacy. Specifically, all client information gathered during Rehab 2 Perform services is protected by Health Insurance Portability and Accountability Act of 1996 (HIPAA). Each employee of Rehab 2 Perform is required to review and acknowledge the HIPAA document and Confidentiality Agreement. Both documents are in effect during employment and post termination.
    Under federal law, you are entitled to a safe workplace. If you have concerns, you have the right to speak up about them without fear of retaliation.

    By signing this document I understand and acknowledge the HIPAA Policy and information, and successfully passed the HIPAA Review. In addition I have read and understand the Safety (OSHA) and Confidentiality Procedures, and watched the OSHA training video.
