Bank Setup Process:
1) Manager completes this form on behalf of themselves and the treasurer.
2) Manager and treasurer attest that they are NOT releated by blood or marriage (even if no longer married or if common-law).
3) Manager and treasurer attest that they have BOTH reviewed the Manager's Page on the website, with particular emphasis on the financials' pieces, including applying for gaming licences, accepting sponsorships or donations from businesses, and reporting requirements.
3) This Jotform will be submitted to CMHA Treasurer who will review. If everything is in order, the form will be sent to the bank.
4) Once the bank is ready for documents to be signed, the bank will reach out to the manager and treasurer.
5) Manager and treasurer will go into the bank for wet signatures. They do not need to go together at the same time.
Please note this change effective the 2025-26 season:
Online access is no longer provided to team accounts. This is due to a change in bank policy. For account balances, teams will need to request a bank statement. CMHA will cover the costs for one statement per month (Sept-March) for each team (7 maximum). This expense will be claimed in the season-end reimbursements.
