    Without this document you will have to pay or ask to waive our Equipment Protection Plan on each rental. If you do have general liability insurance, please be sure to provide a certificate of insurance. See below for our requirements.

     

    1. The Certificate Holder should be:

    2. The Certificate Holder is to be named as additional insured and as loss payee.

     

    3. The certificate of Insurance should show the following:

    a. General Liability: $1,000,000.00 occurrence limit / $2,000,000.00 aggregate.

    b. Auto Liability: $1,000,000.00 single combined limit.

    c. Loss Payee with coverage to include Leased or Rented Equipment. Limit of              coverage to be equal to or greater than Replacement Cost Valuation of rented          equipment.

    d. Rental Vehicles: Hired Liability & Hired Physical Damage to include Leased or Rented Vehicles of the GVW of the rented Vehicle.
