In order to collect personal information about your child, your consent is required in compliance with law prior to your child completing the personal information required on the Jane Doe No More on-line registration.

The personal information we collect may include your child’s first and last name, , address including email or text number, photo, or video of the child, program interest and records of the child’s visits to the Jane Doe No More website and any other information you or your child may provide in a volunteer or participant registration form or if you email or text Jane Doe No More or any of its volunteers

Any personal information provided to us from a child is specifically used for the child’s relationship with Jane Doe No More. Jane Doe No More staff and volunteers may have access to the child’s personal information as part of the provision of the program or service selected by the child. Jane Doe No More will use reasonable means to maintain the confidentiality of such information. At no time is any information collected on a child sold to any third party for any commercial purpose.

Release Waiver of Claim/Data Protection & Privacy Statement
I, the undersigned, hereby release the instructors and members of the organizations known as “East Coast Training Systems: and “Jane Doe No More” from any and all claims of any nature whatsoever stemming from my participation, or the participation of the group I represent, in the above named training activity including, but not limited to, personal injury that I may sustain and I further waive my rights at law to file any further claim. I understand and agree as a participant, or responsible adult in the case of an activity that includes minor children in my charge, in the above stated activity, to release the instructors, all assistants, the above named facility and its employees, and all fellow participants from any and all actions and liabilities, claims, or suffering now and unknown which may be sustained by me, or those in my charge, regardless of any negligence or other action and conduct. I, the undersigned, assume all risks. In consideration of the furtherance of your purposes, objectives, and work, and in consideration of your permitting me to participate in the above named activity, on behalf of myself, my heirs, executors, administrators and assigns, and those in my charge, I hereby waive and release any and all rights and claims for damages that I, or those in my charge, may have or incur during participation in the above named activity, or any activity associated with it, against you, the facilities in which the activity is held, as well as any other person or organization connected to the above stated activity, East Coast Training Systems and Jane Doe No More, it’s owners and instructors, their heirs, executors, successors and assigns, for any and all injuries that I, or those in my charge, may suffer while participating in the above stated activity, and any time before and after said activity. I also release the rights, without further compensation to me, to any photographs or video in which I, or those in my charge, may be a part of, either as the subject or in the background, to East Coast Training Systems & Jane Doe No More, for promotional purposes (website, flyers) which are decent and relate to the services provided by East Coast Training Systems and Jane Doe No More.

Photo/Media Release
I hereby expressly agree, without any right to inspect or approve finished product or use of images, to permit:

1. The taking, use, reuse, publishing and republishing of photographic, including audio and or video recordings and images of me including images, videos or recordings in which my image or voice may be included in whole or in part, or composite or distorted in character or form without restriction as to changes or alterations (editing) from time to time, in conjunction with my name, or fictitious name, or reproductions of such photographs or audio or video recordings made through any medium, and displayed by Jane Doe No More and East Coast Training Systems in any and all media nor or hereafter known, including the internet, for educational, advertising, fundraising or any other lawful purpose;

2. The use of any published content/matter, including statements from you made in conjunction with such photographs, or video or audio recordings; and

3. The digital compositing, or distortion of the images or recordings, including without limitation any changes or alterations as to color, size, shape, perspective, context, sound quality, clarity or content, foreground or background; all in the furtherance of the mission of the Jane Doe No More, Inc. (including advocacy or lobbying).

I hereby expressly assume responsibility for and fully release and hold harmless, Jane Doe No More and East Coast Training Systems, its officers, employees, volunteers and agents, including any photographer, videographer, web editor, engaged by any of them, from and against any claims for damage or loss relating to any of the foregoing.
