This form has all of the questions I need to know in order to schedule you. With all of the information sent up front, it will make the scheduling process way faster for the both of us, so I can get you on my books as soon as possible!

I'm located at Dave Bautista's tattoo shop, DC Society Ink at 1108 W. Kennedy Blvd in Tampa, FL and periodically do guest spots at Red Oaks Tattoo Company in Madison Heights, Michigan! 😊
