I confirm that I am voluntarily choosing to undergo this piercing procedure. I fully understand the nature of the procedure, the risks involved, and the aftercare requirements. I confirm that I am not under the influence of alcohol, drugs or any other substance that may impair my judgement. I understand that should I choose to withdraw from the piercing prior to it being performed, I will still be responsible for paying the remaining fee due for the appointment.

I hereby certify that I have truthfully represented to Bethan Sullivan and representatives, working out of No Morals Limited trading as NMT, that I meet the legal age required for the piercing I am obtaining, and I have provided valid Government issued identification as proof.

I hereby certify that I have disclosed all relevant health information, and that the information provided is accurate and true. I understand that failure to disclose any pertinent health information may result in complications before, during, or after the procedure.

I acknowledge that it is not reasonably possible for Bethan Sullivan working out of No Morals Limited trading as NMT to determine whether I might have an allergic reaction to any of the products used during the procedure, the body jewellery or the recommended aftercare and I agree to accept that this risk is a possibility.

I agree to follow the aftercare instructions provided to me as I acknowledge that infections are always a possibility as a result of obtaining a piercing, particularly if I do not take proper care of my piercing by following the correct aftercare procedure.

I understand that body piercing can result in nerve damage and that if I choose to remove my body jewellery, permanent holes or scares may be left.

I understand that all services fees and jewellery are non-refundable.

I understand there is a chance I may feel lightheaded or dizzy during or after being pierced. I agree to immediately notify Alice Johnson in the event I feel lightheaded, dizzy and/or faint before during or after the procedure.

I understand and agree that once the piercing procedure is complete and I leave the premises, I am solely responsible for the maintenance, security, and care of the jewellery. Bethan Sullivan and No Morals Limited trading as NMT are not liable for any issues that may arise with the jewellery after I leave, including but not limited to loss, damage, or loosening of the jewellery. No replacement will be provided free of charge unless the jewellery is found to be defective at the time of the procedure. In the event of a manufacturing defect, the matter will be handled directly with the jewellery manufacturer.

I agree that Bethan Sullivan is required to take pictures of the piercing results, and I permit them to be used on social media and for marketing purposes, by Bethan Sullivan and No Morals Limited trading as NMT. Should I desire to revoke permission for their use in the future, I understand that I must notify Bethan Sullivan in writing and allow 30 days to accomplish this removal.

I agree to release and forever discharge and hold harmless, Bethan Sullivan, No Morals Limited trading as NMT, the artists, employees & representatives, from any and all claims, damages or legal actions arising from or connected in any way with my piercing, aftercare or procedures and conduct used to perform my piercing.

I acknowledge by signing this form, that I have been given the full opportunity to ask all questions which I might have regarding the procedure, from Bethan Sullivan working out of No Morals Limited trading as NMT, and all my questions have been answered to my full and total satisfaction.

I specifically acknowledge that I have been advised of the facts and matters set forth above, and I am totally satisfied and agree to have the procedure carried out by Bethan Sullivan working out of No Morals Limited trading as NMT. I have entered this release freely or voluntarily and I agree to be bound by the matters set forth above thereby.
