Submit this completed form by January 31, 2025. If your chapter is unable to find a qualified recipient by January 31, we are happy to grant an extension. Please send extension requests via email to chapter.engagement@afpglobal.org. The scholarship will be awarded directly to the AFP ICON registration and will not be sent to the chapter. All scholars must register themselves for AFP ICON online using a special comp code that will be provided to the recipient via email within one week of this form's submission.
