Release:
In consideration of my participation as a volunteer in the Catholic Committee on Scouting Annual Retreat, I hereby hold harmless, indemnify, release and forever discharge the Archdiocese of New York, the Archbishop of New York, Department of Youth Faith Formation, its trustees, directors, officers, agents, employees, and their successors and assigns (hereinafter referred to as “Releasees”) from and against any and all claims, demands, actions and causes of action whatsoever including, but not limited to, claims for property damage, personal injury and death, which are related in any manner whatsoever to this activity excluding claims occasioned by the Releasees negligence. Said indemnity shall include reasonable costs and legal fees incurred by the such organization in defense of any such claims, demands, actions, causes of action, judgments, suits and debts. I agree that photographs, pictures, video, or other media coverage of me may be taken in connection with my participation in the volunteer activity and consent to the use of such photographs, pictures, videos, or other media coverage on Department of Youth Faith Formation’s website or other materials produced by Department of Youth Faith Formation.
