WAIVER AND RELEASE OF LIABILITY FORM

By signing this document, you will waive certain legal rights, including the right to sue.

If this form is not signed, you are not permitted on the premises.

The individual signing below acknowledges and agrees to the following on behalf of themselves and their accompanying group members, including clients, family, etc.

I further acknowledge and agree as follows:

I unserstand that Olga Pavlova Photography Inc. agrees to provide the space in good working order, but they make no unique guarantees as to the studios functionality or suitability to the renters needs.

I agree that Olga Pavlova Photography Inc. is not liable for acts out of its control that affect rental, such as government mandated lockdowns, power outages, construction, weather, emergencies, acts of God etc.

I recognize that there are inherent risks associated with my studio session, and I assume full responsibility for personal injury to myself and (if applicable) my group members. I release and discharge Olga Pavlova Photography Inc. from liability.

I agree that Olga Pavlova Photography Inc. holds no liability for any damage or injury caused by the use of the studio or rental items to me or any third party. I assume all risk of personal property damage or personal injury. If an accident involving the studio's rental items has occurred while it is in my possession, I shall make Olga Pavlova Photography Inc. aware by a written statement of details of the occurrence of the event, including a police report and names and addresses of witnesses, if applicable.

I agree Olga Pavlova Photography Inc. will not be held liable for any injuries or accidents to me, my employees, contractors, clients, or any and all visitors that occur within or outside the Studio premises.

I agree to be liable for any physical damages, legal actions, and/or loss of reputation or business opportunities that Olga Pavlova Photography Inc. may incur as a consequence of my actions or any of my group members while I am in control of the Studio, and shall indemnify and hold harmless Olga Pavlova Photography Inc. against any and all legal actions which may arise from my use of the venue.

I understand that all items must be brought into the studio rooms for myself and (if applicable) my group members. Olga Pavlova Photography Inc. is not responsible for loss or damage to items left in the common areas. I take full responsibility for any items brought with me to the studios.

I agree that if any protocols/terms/conditions etc. are broken, Olga Pavlova Photography Inc. does not have to refund any monies paid for past or future bookings and I may be subject to additional financial or legal penalties.

I agree to pay the full replacement cost of any decor piece, furniture, or other studio item that is damaged or broken during my booking. The cost will be determined by Olga Pavlova Photography Inc. based on the current retail price of the item.

I, the undersigned, affirm that I am of the age of 18 years or older, and that I am freely signing this agreement. I certify that I have read this agreement, that I fully understand its content and that this release cannot be modified orally. I am aware that this is a release of liability and a contract and that I am signing it of my own free will.

I hereby declare that I have had the opportunity to seek independent legal advice with respect to the matters addressed in this document.
