I/We are the legal parents/guardians of the above mentioned child and acknowledge the above information is correct and agree it can be used on the club's website and by managers/coaches for formulation of parent contact lists etc. Team lists and match reports of you and/or your child may be published on the club website or social media sites but will not be identified by thier name. The club fosters an inclusive culture which welcomes community involvement. Whilst not mandatory, there is an expectation of all parents to assist around the club with setup of equipment at training or game day, canteen duty or other duties as required.

IMPORTANT:
- Please note that your child is expected to take part in fitness and training drills that require moderate to high levels of physical exertion. Please ensure you address any issues that may prohibit your child meeting this expectation with your coaches and the club and ensure, that if required, your child both carries and is able to self administer the appropriate medications if left unattended at the club by a parent/guardian.
- You will accept the selection decsions made by the club by playing in the team in which your child is placed

Please note that all EOI's (except Girls Minnie-Tillies Program), are to be accompanied by a $100 deposit which will be deducted from the registration fee balance if your child is successful in obtaining a spot in one of our teams. If your child is unsuccessful, a FULL REFUND of this deposit will be made. Please note that a refund will NOT be issued for change of mind once your child has been alloctad to team and accepted.
