- I accept this risk and hereby release Destination Dance, it’s agents and employees from all liability for personal injury, illness, or properly damage occurring during instruction or performance.
- I certify that my child is in good health and capable of participating in all of the activities and classes.
- I fully understand that demonstration of unacceptable standards of behavior will result in the dismissal of my child from the studio with no tuition refund.
- Destination Dance has my permission to take photos, videos and/or films of my son or daughter and consent to the use of such materials for promotional purposes by Destination Dance.
