I release Z Fit Fitness LLC , Zavian S.J. Burns, its employees or volunteers from any demands, damages, actions or causes of action, either at law or equity of every nature and kind. I understand Z Fit Fitness LLC is not equipped to handle any medical situations or emergencies. I understand that Z Fit Fitness LLC is not a licensed restaurant and the employees are not licensed chefs. I asked Z Fit Fitness LLC to prepare meals for me and any one I distribute meals to. Any sickness I encounter or any injuries I sustain, I will be responsible for. I have the approval of my physician. I agree to participate at my own risk with no liability to Z Fit Fitness LLC, Zavian Burns, its employees or any volunteers. I understand the terms of the agreement and I verify the above information as being true and correct.
