This is the first step to requesting an appointment at Relax Nail Bar & Salon with Molly Mitchell.This Digital Consultation Form will give Molly an idea about your specific hair needs and request to determine if you are a good for for extensions! Please complete the form in its entirety, including photos to best determine the price quote and services customized to you. Price quotes and responses will be processed by the following Monday in which your request was sent. Please keep in that it could be up to 4 weeks that your install appointment can be scheduled. Filling out this form does not confirm your appointment. Extensions are considered a luxury service and require diligent care from behind the chair at the salon and also at home care. Upon completion of your extension application in the salon you will be gifted a starter kit of recommended products and tools to be used on your extensions.

Maintence appointments for extensions are every 6-8 weeks and this is a requirement to ensure proper tension of the natural hair strands on your head, to prevent breakage and scalp damage.

Please be sure when considering if this service is right for you that this is something that sounds doable to you, from a financial and time commitment stand point.

To reserve a consulation a $50 non-refundable fee is required to reserve your consultation appt. which will then go towards your initial install.
