Thank you for allowing us to Treat you and your littles. This application is suitable for kids school lunch boxes . Please fill out the application carefully. If you have any concerns or questions, please do not hesitate to reach out to us. (580) 334-2154
If you order more than one lunch box, please fill out a separate JotForm for each child to help us stay organized or if you have multiple children and want the same please add each child's name and teacher in the notes area. Make sure your child knows they have a lunch box so they can head to the office to pick it up before lunch. Parents, please also inform your child’s teacher that they will be bringing their lunch. We want to make sure every lunch gets delivered, so your help in communicating with the your child & teacher is really important!
Delivery or pickup.

All lunch boxes will be delivered starting at 9:30am for FREE unless you would like to pickup please TEXT me. (580) 334-2154. Please note that every box is custom to your child a lot of time & product go into each & every lunch box and with cost of things it’s hard to keep cost down. If you order more than one lunch box, the price for each lunch box is reduced to $10.00. All other boxes are $15 each, unless you order over 50 lunch boxes, in which case different pricing may apply.
