Clarke/Gray Hockey Liability Waiver

Waiver and Release of Liability:

I, the undersigned, hereby acknowledge and agree to the following:

1. Assumption of Risk: I understand that participating in ice hockey involves inherent risks, including but not limited to physical injury, damage to property, and other hazards. I voluntarily assume all risks associated with participation in ice hockey sessions.
   
   
2. Release of Liability: I hereby release, waive, and discharge Clarke/Gray Hockey (Glen Clarke, Anthony Gray), its officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me while participating in ice hockey sessions.
   
   
3. Medical Treatment: In the event of an emergency, I authorize Clarke/Gray Hockey (Glen Clarke, Anthony Gray) to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for my immediate care. I agree that I will be responsible for payment of any and all medical services rendered.
   
   
4. Indemnification: I agree to indemnify and hold harmless Clarke/Gray Hockey (Glen Clarke, Anthony Gray) from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, arising out of my involvement in ice hockey sessions.

Acknowledgment of Understanding:

I have read this waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
