  • Hey, thanks for your interest in my tattoo services! Here are a few important things to know before we get started, just to make sure we're on the same page:

    • No Color or Tribal Tattoos: I focus on black and grey designs, so I do not work with color or tribal styles.
    • Age Requirement: You must be at least 18 years old to get tattooed. If you're 16 or 17, parental consent is required.
    • Custom-Only Designs: I do not copy other artists’ work. Every tattoo I create is 100% custom, based on the ideas and references you provide. My custom designs are created exclusively for clients who book tattoo appointments with me, and I don’t sell designs separately. This ensures each piece remains unique and personal to those who wear them.
    • Booking Options:

    - Full-Day Sessions (8 hours): $1000
    - Half-Day Sessions (4 hours): $500
    - Smaller tattoos: Occasionally, I can fit these in; Rate at $125/hour (under 4hrs)

    • Retainer Fee: A $40 non-refundable retainer is required to secure your booking.
    • What Happens Next: Once you fill out the form, I’ll reach out via SMS within 5-7 days to discuss your ideas and give you a time estimate for your tattoo or sleeve.

  • Please note that if you are not ready to pay the $40 retainer fee, your request form will be denied. The retainer fee is required to secure any appointment, and this policy applies to all future bookings.

  • Appointment Policies

    To ensure a smooth and professional experience, please carefully review the following:

    • No-Show & Cancellation Policy:
      Clients are allowed a maximum of two no-shows or cancellations. After this limit is reached, you will no longer be eligible to book future appointments with me. Please respect my time and that of other clients by providing sufficient notice for changes to your booking.
    •  Guest Policy:
      Additional guests are not permitted at the appointment unless they have been pre-approved or they are receiving a service. This policy helps maintain a focused and comfortable environment for both myself and the client during the tattooing process.
    • Late Arrival Policy:
      If you arrive late to your appointment, a late fee of $25 will be applied. Please communicate promptly if you anticipate being late. Arrivals beyond 25 mins may result in your appointment being rescheduled or canceled.

    • Please note that once your appointment is booked, the tattoo design is considered final. No changes can be made to the design after booking to ensure we stay within the scheduled time and maintain the quality of your tattoo experience.

  • TATTOO CONSENT & RELEASE FORM

    Acknowledgment of Risks and Consent to Tattoo:

    1. Voluntary Consent:

    • I am at least 18 years of age or 16 years of age with parental consent.

    • I voluntarily request and consent to receive a tattoo from Tamia Kelly at UNCUT INQ studios.

    2. Health Condition Disclosure:

    • I confirm that I do not have any medical or skin conditions, including but not limited to hemophilia, eczema, psoriasis, or allergies, that may affect the healing of the tattoo.

    • I affirm that I do not have any contagious bloodborne diseases such as HIV or Hepatitis.

    • I have not consumed any alcohol or drugs within the last 24 hours and am not under the influence of any impairing substances.

    • I agree to inform the artist of any medical conditions, such as pregnancy or a compromised immune system, before getting tattooed.

    3. Tattoo Design and Placement:

    • I have agreed upon the design and placement of the tattoo and have had the opportunity to discuss it with the artist. I understand that the artist may make necessary adjustments to the design to accommodate my skin’s specific characteristics.

    • I understand that tattoos are permanent and the design cannot be altered once applied.

    • I affirm that I am not requesting any copyrighted or trademarked design unless I have provided explicit written permission from the original artist or owner.

    4. Possible Risks and Complications:

    • I understand that there are risks associated with receiving a tattoo, including but not limited to infections, allergic reactions, scarring, and keloid formation. I understand that these risks may occur and that proper aftercare is essential to minimize these risks.

    • I acknowledge that the tattoo may fade or change over time and may require touch-ups, for which I will be responsible.

    5. Release of Liability:

    • I release Tamia Kelly and UNCUT INQ studios, their agents, employees, and affiliates, from all liability for any injury, loss, or damage that may occur during or after the tattooing process, including but not limited to complications from allergic reactions, infections, or the application of the tattoo.

    • I assume full responsibility for any consequences that may arise from failing to follow the aftercare instructions provided by the tattoo artist.

    6. Photographic Release:

    • I give permission for the tattoo artist and studio to take photographs of my tattoo for promotional, marketing, and portfolio purposes. I understand that these images will not be shared with any third parties without my consent, and my identity will not be disclosed.

    7. Retainer Fee and Payment:

    • I understand that a non-refundable retainer fee of $40 is required to secure my appointment. I acknowledge that the full-day rate is $1000 and the half-day rate is $500, with payment due upon completion of the tattoo session.

    8. No Guarantee on Cover-Ups:

    • For cover-up tattoos, I understand that complete coverage may not always be possible, and the results may vary depending on the existing tattoo and the design requested.

    9. Cancellation and Rescheduling Policy:

    • I agree to notify the studio at least 48 hours in advance if I need to reschedule or cancel my appointment. Failure to do so may result in the forfeiture of my retainer fee.

    10. Aftercare Instructions:

    • I have been provided with and understand the aftercare instructions for my tattoo. I agree to follow these instructions to ensure proper healing and to minimize the risk of infection or complications.

    I confirm that I have read, understand, and agree to the terms and conditions outlined in this consent form. I voluntarily consent to the tattoo procedure and release the tattoo artist and studio from any claims, actions, or liabilities that may arise as a result of this tattoo.
