  • Thank you for being a part of the ART Soccer Center family!

    Please allow 2 business days for someone from our team to reply. If the form is submitted within 2 business days of the next scheduled payment, you may be subject to that payment. If you have multiple children, you must fill out a form for each. 

    CANCELLATIONS - Will take effect 30 days after submission of form; if the billing date occurs before that 30 days, you are still responsible for that payment.

    - If deciding to come back after canceling, you will be subjected to the current membership rates at that time and a $50 reinstatement fee

    - If cancellation form is submitted during a freeze period, the 30 days will begin on the first day membership is set to resume.

    FREEZES will take effect on the upcoming billing period, and billing will resume immediately after the freeze period. (ex. if you choose to freeze your account on the 12th, you will have the rest of the month to complete your sessions and the freeze will begin on the 1st of the following month.)

    • There will be a $15 charge to your account for each month your membership is in the freeze period to reserve your membership
    • You can come back early from a freeze
    • The minimum months to freeze your membership is 1 month and the maximum is 6 months. You can use up to 6 freezes throughout the year.
    • Billing will begin on the 1st of the month after the freeze ends. 

    UPGRADING / DOWNGRADING ACCOUNT - This form will act as a replacement to your current agreement. Please note that membership changes made during the presale promotion will be honored with our presale promotional prices. If you plan to make a change to your account after the presale promotion ends, you will be subject to the increased rates.
