Student ID Request Form

Please Read

Please allow two weeks for ID to be mailed to your school counselor. You will keep the same ID each year you are dually enrolled at Coahoma. Replacement IDs cost $25.

You may also send a cashier check or money order; however, be sure to include the Memo: For Student ID and include your student ID number (for example: Memo: For Student ID 1864578). The cashier check or money order will be made out to Coahoma Community College and mailed to:

Please read the following rules regarding uploading a photo for your ID:

You must stand in front of a smooth, plain, solid, white, or cream wall with nothing showing behind you. You are not to wear sunglasses, masks, hats, hoods, visors, night scarves, bandannas, sleeveless shirts, or anything that may cover the face, gang-related, offensive, or too revealing. No camera filters are allowed, and no hand gestures are allowed.

ID picture may be used as your yearbook picture, so casual dress is required. Please face the camera straight ahead when taking the picture for the ID. No picture with you not facing directly to the camera will be accepted.

Only a simple smile or straight face will be accepted. No selfie or glamor-shot style pictures will be accepted.

Failure to follow the above instructions will result in ID photo rejection.
