Instructions for Nominating:

To nominate an team, please fill out the form and include as much background information as possible. This must include pictures, newspaper clippings, etc.

Please review the entire form before filling it out to ensure you have all of the information prepared.
Eligibility of Nomination:

TEAM is defined as a group of individuals (two or more) who have competed together to determine the outcome of a recognized provincial, national or international championship, have achieved noteworthy professional success, or have made a significant and unique contribution to the history or heritage of sport in the Town of Riverview. The team must be from the Town of Riverview.
- Teams are eligible for nomination after five (5) years have elapsed since the team last competed as a unit.
- If a “team” has been successfully recognized for an achievement, an individual team member may not be nominated for that same achievement unless additional, significant achievements have occurred.
