Consent to Release and Exchange Information Form

The mission of WeBuild Concord is to be a catalyst for building homes, partnerships, and opportunities for the stability and wealth of our residents. We envision a community where attainable housing is stable and safe and part of our economic and social vision. Thus, WeBuild Concord is not an ordinary housing developer, builder, seller, or renter of homes, but we are a mission dedicated to protecting and providing residents with opportunities to thrive.

To better serve you, WeBuild Concord may need to release personal information about you to one or more of our partners who may be providing services to you (a “Partner”). WeBuild Concord may also receive personal information about you from these Partners related to our services. This exchange of information helps us and our partners serve you better by allowing us to understand your situation and coordinate housing and services more effectively. It also helps us evaluate and improve WeBuild Concord housing placements and programs.

The personal information may include your name, address, phone, or other contact information, employment status, loan, and downpayment application status, downpayment qualifications and amounts, rental history, or information about income, public assistance payments, or other financial matters. It may also include social information related to your housing needs (i.e., medical or legal).

We seek your consent to exchange such information with our Partners regarding your engagement with WeBuild Concord. Client Consent

I consent to WeBuild Concord releasing, exchanging, or receiving my personal information with our Partners for the above purposes. I confirm that any information I provide to WeBuild Concord or our partners is true and accurate to the best of my knowledge. All information obtained by WeBuild Concord will be confidential.

Signature

I have read this document, and I understand its terms. I sign it freely and voluntarily. I understand I am not required to sign this release form to engage with WeBuild Concord, even if some forms of assistance may be limited.
