Sponsor Agreement:
By submitting this form, the company/organization commits to the selected membership amount and agrees to submit the payment by the date indicated in the invoice. This membership is annual from the date that this form is submitted. After the form has been submitted, an invoice will be issued for the amount indicated on the form. NAHJ reserves the right to allocate benefits based on the membership level and payment received.
